Why AI in Diagnosis Is Now a Top Patient Safety Concern

Artificial intelligence has quickly become part of clinical workflow. Now attention is turning to how these tools may influence diagnostic decision-making.

Some NPs use AI to help summarize notes or organize documentation. Others use it to review clinical guidelines, check a differential diagnosis, or think through a tricky case. Even if you personally avoid AI tools, you are probably encountering them indirectly through EHR decision-support features or automated summaries.

As AI tools become more deeply entwined with clinical practice, concerns about their impact on patient safety is growing. The Top 10 Patient Safety Concerns 2026 report from ECRI and the Institute for Safe Medication Practices lists the AI diagnostic dilemma as the number one patient safety concern this year. Not medication errors. Not diagnostic delays. AI.

That ranking raises an obvious question: Why? After all, healthcare has faced plenty of major safety challenges over the years, from medication errors to hospital-acquired infections. What makes AI rise above those concerns now?

Diagnosis Is Already One of the Hardest Parts of Medicine

Diagnostic errors remain one of the most complex and consequential types of medical error. They can lead to delayed treatment, inappropriate therapies, and missed opportunities for early intervention.

AI is often promoted as a potential solution to this problem. In theory, systems that analyze large amounts of clinical information could help identify patterns that humans might overlook.

One concern highlighted in the ECRI report is that heavy reliance on AI tools could gradually weaken those skills. When clinicians routinely depend on AI systems to guide diagnostic thinking, they end up doing less cognitive work themselves. Over time, that reliance could erode diagnostic reasoning skills that are built through repeated practice.

The concern may be even greater for clinicians in training. Trainees who consistently use AI tools to suggest diagnoses or structure clinical reasoning throughout their educations will have fewer opportunities to develop those skills independently.

These possibilities are part of what ECRI refers to as the AI diagnostic dilemma. The technology has the potential to help, but it also introduces new layers of risk.

The Speed of Adoption Is Unusual for Healthcare

Healthcare usually moves slowly when adopting new technology. Clinical tools often go through years of evaluation, training, and gradual implementation before becoming widespread. AI has followed a different path.

In just a few years, AI tools have gone from experimental projects to everyday utilities that clinicians can access instantly. That rapid adoption is part of what has caught the attention of patient safety organizations. This technology has spread quickly through clinical environments, vastly outpacing the development of clear policies, training, and oversight.

In other words, clinicians are already using these tools, but the healthcare system is still figuring out how they should be used safely.

The Cognitive Trap: When AI Sounds More Certain Than It Should

One of the biggest risks associated with clinical AI is automation bias. This occurs when clinicians give disproportionate weight to recommendations generated by automated systems.

AI outputs are often structured, confident, and well explained. A differential diagnosis generated by an AI tool may come with clear reasoning and ranked possibilities. That presentation can make the suggestions feel authoritative. An explanation may sound convincing even when the conclusion is incomplete or incorrect.

Most clinicians do not blindly trust AI. The more realistic risk is that once a tool highlights a few likely diagnoses, it can steer attention toward those possibilities and away from others that might still need to be considered.

AI can process huge amounts of information quickly, but it can’t understand the full context of a patient sitting in front of you. It doesn’t pick up on the social factors that shape a patient’s health decisions, and it doesn’t have the intuition that comes from years of clinical experience.

AI Reflects the Data It Learns From

Most clinicians are likely aware of how AI tools can perpetuate bias. If disparities exist in the training data, they can appear in the AI’s output as well.

Most AI models learn from large collections of medical records, research studies, and other healthcare data. The problem is that those sources don’t represent every patient population equally. Medicine has long struggled with gaps in research and documentation across different groups of patients.

But identifying where those gaps exist in individual tools is next to impossible, due to the so-called black-box problem. Developers of these tools aren’t particularly known for being transparent about the data and methods used to train their models. There’s no easy way to identify which tools might be susceptible to biased outputs.

That means a diagnostic tool might perform very well for some patient populations and less reliably for others. The challenge is that those differences aren’t always obvious when you’re using the tool in the middle of a clinical visit.

The Real Issue Isn’t the Technology

The main concern isn’t just that AI tools exist. That ship has already sailed.

More and more clinical tools are implementing AI-powered features, and that trend is unlikely to slow down. That puts clinicians in an unusual position, as guidance is still developing while the tools are already becoming embedded in practice.

The challenge now is making sure these tools support the diagnostic process without having the last word in deciding the diagnosis.
